An application for a license shall be filed with the city recorder on an official form at least 20 days prior to the date of the dance. The following information shall be given:

A. The full name, date of birth, Oregon driver’s license, Social Security number and address of the applicant, or, if made on behalf of a firm or corporation, the full names, dates of birth, Oregon driver’s licenses, Social Security numbers and addresses of the principal officers thereof;

B. A brief description of the principal business of the applicant;

C. Places and approximate dates of public dances conducted by the applicant during the past five year;

D. A statement as to whether or not the applicant has ever been convicted of a crime other than minor traffic violations;

E. Street address of the place where applicant intends to conduct dances within the city;

F. Security shall be provided by bonded security services licensed by the city under Chapter 5.76 MMC, a law enforcement agency, or a law enforcement reserve organization. (Ord. 4343 §1, 1985; Ord. 3961 §5, 1978).
